Set Appliance State:

Isolate DHW
Drain boiler's primary and hot water circuit
Remove fan
Drain boiler's primary and hot water circuit
Remove fan
Observation:
Remove plate heat exchanger from appliance, through where the fan was located.
Inspect the plate heat exchanger for scale build-up or restrictions.
Note:
Some residual water will be present when heat exchanger is removed. Take precautions to prevent damage to boiler electrics.

Isolate as per TOPs P4
and refit as necessary
