Set Appliance State:

Drain primary circuit of boiler down
Turn off HW Isolation Valve
Turn off HW Isolation Valve
Observation:
Remove secondary heat exchanger and inspect for restrictions/blockages.
If you fill the primary side of the heat exchanger with water, does it flow out the other side quickly. Does the water contain any bits of sludge as it empties?
If you knock the heat exchanger gently with a screwdriver whilst it contains water that may loosen some bits of sludge which is an indication of restrictions.

Isolate as per TOPs P4
and refit as necessary
